Overview

B. breve M-16V is a probiotic strain developed by Morinaga Milk Industry (Japan). It was isolated from the feces of a healthy human infant [1]. It is predominantly studied in neonatal and pediatric populations. Evidence supports its use for prevention of necrotizing enterocolitis (a severe gut disease in premature babies) in preterm infants, adjusting allergic responses, and restoration of gut microbiota following cesarean section [1,2]. M-16V is recognized as safe for use in preterm and term infant populations [1].

Mechanism

M-16V tunes intestinal and systemic immune responses through interaction with toll-like receptors (TLR-2, TLR-4) on dendritic cells. This promotes a Th1/Th2 balance favorable to tolerance induction [1]. The strain reduces intestinal permeability. It attenuates allergic inflammation through TGF-beta-mediated pathways, as demonstrated in mouse models of food allergy and asthma [1,3]. M-16V colonizes the neonatal gut. It promotes the establishment of a Bifidobacterium-dominant microbiota, particularly when combined with prebiotic oligosaccharides (scGOS/lcFOS) [2,4].

Health Benefits

Wong et al. (2019) provide a comprehensive strain-specific review. It documents evidence from over 20 clinical studies. These include RCTs for necrotizing enterocolitis prevention, reduction of allergic symptoms in infants with atopic dermatitis (eczema), and improved microbiota composition in cesarean-delivered infants [1] [review]. Wang et al. (2025) conducted an RCT (n=152). They demonstrated that synbiotic formula containing M-16V restored Bifidobacterium-dominant microbiota in cesarean-born infants by 12 weeks [2] [RCT]. Chew et al. (2024) conducted an RCT (n=40). They showed M-16V plus scGOS/lcFOS in young child formula improved gut microbiota and iron status in healthy toddlers [4] [RCT]. Athalye-Jape et al. (2025) conducted an RCT (n=56 preterm infants). They compared live vs. heat-inactivated M-16V and demonstrated live probiotic colonization patterns [5] [RCT]. Multi-strain RCT evidence (with BB536 and M-63) supports efficacy for seasonal allergic rhinitis in children [6] [RCT, multi-strain].

Biology

Taxonomy: Bifidobacterium breve (valid species per Approved Lists 1980). It is Gram-positive, only grows without oxygen, non-motile, non-spore-forming, and irregular rod-shaped. It ferments carbohydrates via the bifid shunt, producing acetate and L(+)-lactate. It does not ferment pentoses or gluconate, which is a key difference from B. longum. Habitat: infant gastrointestinal tract. It is a human milk-associated species.

Available Products

Morinaga supplies M-16V as freeze-dried powder (M-16V Probiotic Cultures) to infant formula manufacturers globally. It is used in infant formulas (Danone Nutricia Neocate Syneo, Mead Johnson Enfamil), synbiotic preterm and term infant formulations, and hospital neonatal protocols in Japan, Australia, and New Zealand [1,2].
